2) Credit Card Authorization Form by fax

Credit Card Authorization Form (below) is required for all transactions and faxed back to our number + 90-212-638-8764. For security purposes we require a photocopy of your credit card, your signature, billing address and home telephone number. No second-party credit cards will be accepted. Please print out the form below and fax it to our office after filling up the requested information.
